I wrote this book as a follow-up to TMJ: Its Many Faces. Even with the outstanding reception the first book received, I felt more information needed to be written for the general public. Billions of dollars are spent each year for healthcare, much for the diagnosis of pain. Pain sufferers will do and pay anything for some hope of relief. Unfortunately, insurance companies don't share such enthusiasm for pain relief. And it's going to get worse, at least in the United States.
Because most doctors both don't understand nor pursue the numerous causes of pain, the medical profession won't demand reasonable payment from insurance companies for adequate pain treatment. It's funny to see how doctors and insurance executives alike change their attitudes when a loved one is in pain!

In 1983, I conducted a survey of 135 consecutive pain patients. We discovered that our office was the 6th, on average, to be consulted for orofacial pain problems. For one lady from Kentucky, I was her 31st doctor. Can you imagine that? I’ve also been the 29th doctor for one lady from Tennessee. Two common denominators soon became apparent from that survey: |

| 1. The doctors apparently didn’t listen about the patients’ pain complaints; |
| 2. The doctors often didn’t believe the patients’ complaints. |

These commentaries on the medical and dental professions are sad. What’s even worse is many doctors, regardless of degree, don’t even believe in many pain syndromes. How can this be? |

Many doctors don’t believe in pain disorders which weren’t taught when they were in school and residency programs. That’s awful arrogant because so much knowledge concerning pain has been discovered in recent years, many years after of these so-called experts graduated. I’ve been fortunate enough to be directly involved in the discovery and/or research of several pain disorders, none of which were known even as late as 1980. For a doctor to deny validity in a pain disorder not taught to him in school is like a pastor not believing in heaven or hell. It just doesn’t make sense. It’s intellectual suicide. |

I see suffering people every day. They’re just like you and me, but unfortunately, for any number of reasons, they have an extra burden to carry: pain. Perhaps that’s why I’m so cynical about my colleagues who refuse to believe these people are suffering. |

| Here’s an example. This lady from California, sent this conversation with God just to demonstrate to me (she hadn’t yet seen me) her extent of suffering: |
Oh Lord, Lord Jesus, why me? I have suffered long enough, yet I hold on hoping you will guide me to help. I have lost all my loved ones and stand alone with you by my side. Mom always told me to trust in the Lord, and take my troubles to you. I have those that love me and pray for me daily, but the pain is so horrible. It’s so hard to keep believing that there will be a day when this horrible nerve will stop invading my mouth so that I can sing in church and pray with others out loud. It hurts so bad Lord. Please don’t misunderstand my plea, as I know that so many are suffering with all kinds of pain. But Lord Jesus I know I could endure this so much easier and longer if it wasn’t in my mouth. Forgive me for thinking bad thoughts, about not wanting to live, if I have to live with this every waking hour. So many times I think of just ending this pain, and joining you . . . I just keep on having faith that someone will believe me and help me through you. I ask your guidance and faith in the power of prayer. In Jesus’ name . . . amen. Betty L (used with permission) |

See why I felt this book was needed? Although severe, this dear lady isn’t unusual. There are many people like this. Most doctors and insurance companies don’t want to deal with them, so the suffering is magnified. |

In this book, I’ve revisited the subjects of Pain and Neuralgia Inducting Cavitational Osteonecrosis (NICO). We’ve learned more about both these subjects since my first book was published. I included new and more in-depth material about pain, one subject that truly binds us all together. In addition, I’ve written the chapter on NICO both for the public and for health professionals. This horrible disorder of dead bone plagues so many, especially those with facial involvement. |

Subjects such as Odontalgia (tooth pain) and Burning Mouth Syndrome are presented, describing causes and successful forms of therapy. |

In addition, controversial, yet scientifically valid material is presented concerning the systemic effects of periodontal disease. Dr. James Boyd has presented some revolutionary information on clenching, headaches and the treatment of these disorders. |

Lastly, I hope the tone of this book will be encouraging. Most pain conditions, if diagnosed correctly, can be helped. Without Divine intervention, many pain syndromes can’t be cured, but most can be significantly improved. |
